Creating Intake Requests from Forms
Intake forms make it easy to submit standardized requests that automatically inherit template settings and default assignees, streamlining the document creation process.
Step-by-Step Instructions for Creating an Intake Request from a Form
- Navigate to the Intake section from the left navigation panel.
- Click on the Requests tab (if not already selected).
- Click the New intake form request button.
- From the dropdown, select the form you want to use.
- Fill in the required information in the form fields. Any help text provided by administrators will appear beneath the relevant fields. (Suggest screenshot of a form with fields and help text)
- If needed, add attachments by clicking the Add attachment button.
- When you're finished, click Submit to create your request.
- Your request will be assigned to the default assignee (if one was specified for the form) and they will be notified of your submission.
Tracking Your Requests
- After submitting a request, you can track its status from the Requests tab.
- Click on any request to view its details, add comments, or check its progress.
Creating Documents from Requests
Once your request has been reviewed, you or an assignee can create a document based on the request:
- Open the request from the Requests tab.
- Click the Create document button.
- The system will automatically create a new document using the linked template, with all completed fields from your request already populated.

A: Any fields left empty in your request will remain empty when the document is created. Only the information you provide will be populated in the final document.
Q: Can I still access my request if the linked template is deleted?
A: Yes, you can still access your request even if the linked template is deleted, but you will not be able to create a document from it since the template no longer exists.
Q: Who can see my submitted request?
A: The request assignee and users with the "Review and edit all request" permission can see and modify your request. You will always have access to requests you've created.
Q: Can I update my request after submission?
A: Yes, you can update your own requests after submission by opening the request and making changes.
